Job category and statutory minimum wage
Must state the assigned duties (General Service, Cooking, or Caregiving) and guarantee that pay never falls below the legal minimum.
Itemized social security deductions
Law requires itemizing each personal deduction: Pension (15%), FONASA (3% to 8%), and FRL (0.1%). Global lump sums are invalid.
Settled period and worked hours
Must accurately specify the settled month, weekly agreed hours, and days actually worked during the period.
Seniority, overtime, and presenteeism bonuses
Seniority bonus (0.5% yearly cumulative), paid holidays, overtime, or bonuses must be itemized on separate lines.
Mandatory Duplicate Copy (2 identical signed copies)
Each month two identical copies must be issued and signed: one retained by the employer and one delivered to the employee. It is the sole legal proof of actual wage payment.
